29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  Washington St., Somerville, MA   On or about, May13, 2021, employees were exposed to cave-in struck-by hazards while installing sewer pipe in 9.3-foot deep trench with straight earthen walls where the Speedshore aluminum hydraulic shoring was not installed in accordance with the manufacturer's instructions and street end trench wall without any protective system in place.  The C. McGue Landscape Design & Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1254437, citation number 1, item number 1, and was affirmed as a final order on March 6, 2018, with respect to a workplace located at 31 North Beacon St., Allston, MA 02134.
