29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   On or about 05/17/2021, at  5181 NW 108th Ave, Sunrise Florida 33351, at a shipping and receiving warehouse, employees were exposed to struck-by hazards when powered industrial truck operators were driving forklifts without the required training.
