29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about May 25, 2021, employees were exposed to machine hazards associated with steam when they engaged in servicing activities on the Lavender Utility Press/Unity Press. The employer did not ensure that employees implemented energy control application steps prior to allowing them to perform servicing. The machine did not have lockout devices applied to all energy sources prior to the employee engaging in the servicing / maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. (a) On the first floor, the Flat Ironer, American Laundry Machinery Industries (S/N: 155-M-3390) did not have guarding provided to prevent the operator, who is feeding into or receiving from the ironer, from reaching hazardous nip points, or accidentally contacting the uncovered heated surface, or both.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. On the first floor, the Cissell Dryer had an extended projecting shaft with a 1 1/4" diameter which was not smooth and projected 1 1/4" exposing employees to entanglement hazards.

29 CFR 1910.264(c)(4)(iii)(a): All steam pipes that are within 7 feet of the floor or working platform, and with which the worker may come into contact, shall be insulated or covered with a heat-resistive material or shall be otherwise properly guarded. (a) On the first floor, the steam pipes by the American CCW-C had exposed steam pipes exposing employees to thermal burns due to pipe temperature approximately 316 degrees F. (b) On the first floor, the steam pipes by the American Dryer Corp had exposed steam pipes exposing employees to thermal burns due to pipe temperatures approximately 190-203 degrees F. (c) On the second floor, the steam pipes by the Cissell Steamer had exposed steam pipes exposing employees to thermal burns due to pipe temperatures approximately 300 degrees F. (d) On the second floor, the steam pipes by the Ajax Cuff & Collar (Model: CCW, S/N: SPHP2661502) had exposed steam pipes exposing employees to thermal burns due to pipe temperatures approximately 325 degrees F. (e) In the basement, the steam pipe with the screw in the hallway near the men's restroom had an exposed steam pipe exposing employees to thermal burns due to pipe temperatures approximately 200 degrees F.
