29 CFR 1910.141(a)(5):Vermin control. Every enclosed workplace shall be so constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. A continuing and effective extermination program shall be instituted where their presence is detected. (a)Facility- 100 Hafner Avenue, Pittsburgh, PA: On or about May 25, 2021, employees were exposed to injuries and illnesses from vermin such as but not limited to racoons living in the building.

29 CFR 1910.178(l)(1)(ii):Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). (a) Facility- 100 Hafner Avenue, Pittsburgh, PA: On or about May 25, 2021, the employer did not ensure all employees operating power industrial trucks had successfully completed the required powered industrial truck training under paragraph (l).
