29 CFR  1926.502(f)(1)(i): Fall protection systems criteria and practices. Warning line systems. When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge.  a) On or about 05/25/2021 employees were working on a flat-roof while warning lines were hanging from the edge of the building and no additional fall protection system was in place.

29 CFR  1926.503(a)(1): Training requirements. Training program. The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) On or about 05/25/2021 employees have not been trained on fall protection systems and on hazards of falls, while working at heights of  above 40 ft. from the ground.
