29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about May 26, 2021, at the construction site located at 450 Sunny Slope Drive in Hartland, Wisconsin, employees were performing roofing work on a commercial building with a pitch of approximately 10 in 12 and fall height greater than 25 feet without fall protection in place, thereby exposing the employees to a fall hazard.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about May 26, 2021, at the construction site located at 450 Sunny Slope Drive in Hartland, Wisconsin, employees were exposed to a fall hazard of greater than 25 feet and had not been trained to recognized and minimize the fall hazards associated with their work.
