The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or around May 26, 2021, the employer did not ensure that each machine operator and other mill employees were trained to isolate energy sources and render equipment inoperative prior to allowing them to perform maintenance and set up activities on Mill 1.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about June 3, 2021, on Mill 1, each mill employee performing setup activities, did not lock out the electrical disconnects when performing tasks such as cleaning and changing out rolls. Each employee was not required to: (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (3) affix lockout or tagout devices to each energy isolating device, (4) verify effectiveness of energy isolation.

Before lockout or tagout devices are removed and energy is restored to the machine or equipment, procedures shall be followed and actions taken by the authorized employee(s) to ensure the following: The work area shall be checked to ensure that all employees have been safely positioned or removed. a) On or about May 26, 2021 when the machine was in jog mode during the setup process, the work area was not checked to ensure employees were in safe position prior to jogging the machine, resulting in an employee's hand being pulled into the sizing rolls.

In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment, or component thereof, the following sequence of actions shall be followed: Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; a) On or about May 26, 2021, after release from lockout while the machine was in job mode during the setup process the work area was not checked to ensure employees were in a safe position prior to jogging the machine with the remote, resulting in an employee's hand being pulled into the sizing rollers.
