29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  (a) T&T Ventures dba Lightning Exteriors  at  4914 - 4964 Jericho Way in Rapid City, SD 57702: On or about and at times prior to June 2, 2021,  the controlling employer did not initiate and maintain a safety program which provided for frequent and regular inspection of roofing jobsites, materials and equipment. Subcontractor employees were permitted to work in areas where fall hazards existed without the use of a fall protection system. This condition exposed subcontractor employees to fall hazards greater than 7 feet.
