29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  Field Site: On or about May 24, 2021, the employer did not ensure employees engaged in construction activities in a cellar at the Chevron Delaware Pad 13 site,  were protected from falling by a fall arrest system or guardrail system.  This condition exposed employees to a fall hazard above 6 feet.
