29 CFR 1926.501(b)(4):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: Worksite located at 1920 Cataline Ave., Lee's Summit, Missouri- Employees were exposed to a fall hazard in that the employer had cut a large hole for the stair case to be located and had failed to cover the hole to prevent employees from falling through the hole.
