29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person(s).  The employer is failing to ensure that employees are protected from fall hazards while performing roofing activities.  This violation was most recently documented on or about June 4, 2021 at a jobsite located at 3730 N 56th St., Lincoln Nebraska.  On or about May 29, 2021, an employee was working on the roof of a commercial building when he fell through a skylight fixture.  This hazardous condition was not appropriately identified and eliminated by a competent person prior to work commencing.

29 CFR  1926.754(e)(3)(iv):Smoke dome or skylight fixtures that have been installed, are not considered covers for the purpose of this section unless they meet the strength requirements of paragraph (e)(3)(i) of this section.  The employer is failing to protect employees from fall hazards that are associated with working around skylight fixtures that are not capable of supporting, without failure, twice the weight of the employees, equipment and materials that may be imposed on them at any one time.  This violation was most recently documented on or about June 4, 2021 at a jobsite located at 3730 N 56th St., Lincoln Nebraska.  On or about May 29, 2021, an employee was working on a roof of a commercial building when he fell through a skylight fixture that failed to support the required weight specification.

29 CFR  1926.761(b):Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.  The employer is failing to train all employees that were potentially exposed to fall hazards to recognize the hazards associated with the type of work being accomplished. This violation was most recently documented on or about June 4, 2021 at a jobsite located at 3730 N 56th St., Lincoln Nebraska.  On or about May 29, 2021, an employee was working on a roof of a commercial building when he fell through a skylight fixture.

29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  The employer is failing to report in-patient hospitalization of one or more employees as a result of a work-related incident to OSHA within twenty-four (24) hours.  This was most recently documented on or about June 4, 2021 at a jobsite located at 3730 N 56th St., Lincoln Nebraska.  On or about May 29, 2021, an employee was working on the roof of a commercial building when he fell through a skylight fixture, resulting in an in-patient hospitalization that was not reported within the required time frame by the employer to OSHA.
