Workers were required to swim across a pond, resulting in the drowning of a worker.  b) At the vicinity of 6820 Cold Brook Road, Scott, NY ., on or about June 1, 2021 and thereafter: The employer did not train workers to recognize and avoid electrical hazards when installing fiber optic lines on pre-existing electrical poles  . Work was performed within 5 feet of 4.8 KV overhead electrical lines without any protective controls in place.
