On or about June 7, 2021, at the above addressed jobsite, the employer did not ensure that an inspection of the Fluke 1587 FC Insulation Multimeter S/N 40090026 was conducted by a competent person.

29 CFR  1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a.  On or about June 7, 2021, at the above addressed jobsite, employees performing bus duct installation operations on the 55th floor were not protected against electric shock by de-energizing and grounding the circuit or by guarding it effectively by insulation or other means, thereby exposing the employees to electrical hazards.

29 CFR  1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized:  a.  On or about June 4, 2021, at the above addressed jobsite,  an employee visually verified that the "Bus Duct A4" SIEMENS 1600 Amp breaker was de-energized, but did not render it inoperative and tag it to prevent re-energization, thereby exposing employees to electrical hazards, including, but not limited to, arc flash hazards.
