29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. On or about June 7, 2021, at the above addressed jobsite, employees performing bus duct installation operations on the 55th floor were not protected against electric shock by de-energizing and grounding the circuit or by guarding it effectively by insulation or other means, thereby exposing the employees to electrical hazards.

On or about June 4, 2021, at the above addressed jobsite, an employee visually verified that the "Bus Duct A4" SIEMENS 1600 Amp breaker was de-energized, but did not render it inoperative and tag it to prevent re-energization, thereby exposing employees to electrical hazards, including, but not limited to, arc flash hazards.
