29 CFR  1926.106(a): Employee(s) working over or near water where the danger of drowning existed, were not provided with U.S. Coast Guard approved life jacket(s) or buoyant work vest(s):  On or about 3 June 2021, at Loiza Shoreline Protection Project, Puerta del Atlantico Street, Loiza, PR.  At the shoreline, an employee while operating an excavator by placing rocks and marine mattresses was working over water without wearing a life jacket.    NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR  1926.601(b)(4): The employer had in use motor vehicle equipment having an obstructed view to the rear which did not have a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to back up the vehicle:  On or about 3 June 2021, at Loiza Shoreline Protection Project, Puerta del Atlantico Street, Loiza, PR.  At the worksite, the employer allowed the entrance of a dump truck full of heavy rocks going backwards with no backup alarm nor a ground guide/observer to direct him on the ground.    NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
