This was most recently documented on June 8, 2021 at the jobsite located at 2824 86th Street in Lincoln, NE. Two employees were performing roofing activities on the roof of a new construction home and were not utilizing fall protection.

This was most recently documented on June 8, 2021 at the jobsite located at 2824 86th St in Lincoln, NE. One roofer used nail guns while performing roofing activities and did not use any eye or face protection.

This was most recently documented on June 8, 2021, at the job site located at 2824 86th St in Lincoln, NE. The employer failed to ensure that where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails extended at least 3 feet (.9 m) above the upper landing surface being accessed.
