29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  The employer is failing to protect employees from fall hazards while performing equipment installation activities. This was most recently observed at 6554 S. 200th Ave, Omaha, Nebraska on June 08, 2021 where the employer has employees installing communication equipment on a residential home without fall protection where the ground to eave was over 10 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  The employer is failing to protect employees from fall hazards while performing residential roofing activities. This was most recently observed at 6554 S. 200th Ave, Omaha, Nebraska on June 08, 2021 when the employer has employees using a ladder whose side rails did not extend at least 3 feet past the roof eave and the ladder was not secured.
