29 CFR 1910.29(b)(13)(i): The guardrail system opening used around holes that serve as points of access (such as ladder ways), did not have a self-closing gate that slides or swings away from the hole, and was equipped with a top rail and midrail or equivalent intermediate member that met the requirements in paragraph (b) of this section:  (a) On or about June 8, 2021 and at times prior, for the employees exposed to an approximate seven foot fall hazard where a self-closing gate was affixed in the open position on the Node three catwalk access point located at 5757 Wayne Newton Blvd., Las Vegas, Nevada.
