29 CFR  1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a) WellAge GJ Employer LLC dba The Lodge at Grand Junction, located at 2656 Patterson Road, Grand Junction, CO 81506: On or about June 3, 2021, the employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace when changing residents oxygen tubing and monitoring use of equipment. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing respiratory care to residents. This condition exposed employees to a respiratory hazard.

29 CFR  1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):   a) WellAge GJ Employer LLC dba The Lodge at Grand Junction, located at 2656 Patterson Road, Grand Junction, CO 81506: On or about June 3, 2021, the employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT) before being assigned to wear a respiratory during assigned tasks including changing residents oxygen tubing and monitoring use of equipment. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing respiratory care to residents. This condition exposed employees to SARS-CoV-2 virus hazards.
