29 CFR  1910.178(o)(1):The employer did not ensure that only stable or safely arranged loads were handled:  a) Yard - On or about June 2, 2021, the employer failed to ensure that only stable or safely arranged loads were handled.  A forklift operator was handling several bundles of steel beams on an uneven gravel surface causing the load to become unstable and fall. Thereby exposing employees to struck-by hazards.
