29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  JOB SITE: Corner of Highland Dr and Westwood Blvd, Poplar Bluff, MO 63901  On or about 6/10/2021, workers were observed  on top of the roof actively installing plywood roofing material.   A worker was operating pneumatic nail gun without any form of eye or face protection.  The worker was exposed to struck-by hazard.

29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  JOB SITE: Southeast corner of Westwood and Highland Dr., Poplar Bluff, Mo 63901  a)	On or about 6/10/21, workers were observed on top of flat roof, approximately 21 feet from ground level, actively installing roof decking material. The workers were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures. The workers were exposed to a fall hazard.  b)	Workers standing in a container located on the forks of the industrial fork truck while handling decking material were not protected from falling by guard rail system or personal fall arrest system, employees were exposed to falling 20 feet to the lower level without guard rail system or personal fall arrest system.  There was not a guardrail on the front of the container.  All 5 employees were lowered to ground by employer while riding in container.

JOB SITE: Southeast corner of Westwood and Highland Dr., Poplar Bluff, Mo 63901, on or about 6/10/21, workers and  roofing material were observed on platform raised by an powered industrial truck(10054 Skytrak) , approximately 20 feet from ground level, not secured to the forks of the powered industrial forklift.  The workers were exposed to a fall hazard.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.   JOB SITE: Southeast corner of Westwood and Highland Dr., Poplar Bluff, Mo 63901  On or about 6/10/21, workers were observed on top of flat roof, approximately 21 feet from ground level, actively installing roof decking material.   The ladder to gain access did not extend three feet past the upper landing surface.   The ladder was contacting lower on the top of the 15 foot ceiling wall.  The workers were exposed to a fall hazard.
