29 CFR 1926.652(c)(3)(iii):At least one copy of the tabulated data, which identifies the registered professional engineer who approved the data, was not maintained at the jobsite during construction of the protective system: On or about June 9, 2021, employees were exposed to a trench box malfunction while repairing sewer lines at a residential home located at 2511 Highland Cir., Ashtabula, Ohio 44004. The employees were working in a trench that was protected from a cave in by a trench box and the employer did not ensure that he a written copy of the tabulated data for the box at the site.
