29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 11, 2021, on the North side of a two-story multi-family dwelling, employees were exposed to a fall hazard while using an extension ladder to access a roof and the second level balcony without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing or other rigid support.

On or about June 11, 2021, and times prior thereto, on the roof and exterior area of a two-story multi-family dwelling that was being renovated, employees were exposed to eye injuries when operating pneumatic nail guns without eye protection.
