29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Jobsite: On or about June 15, 2021, nine employees were not protected from falling to the ground while they were installing an asphalt shingle roof on a commercial building. The fall hazard ranged from approximately 14 to 30 feet.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite: On or about June 15, 2021, the employer did not provide fall protection training to its employees who are engaged in roofing work. Nine employees were not protected from falling to the ground while they were reroofing a commercial building.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite: On or about June 15, 2021, both portable ladders that employees were using to access/egress the roof of the commercial building only extended 1.5 feet above the edge of the roof. The fall hazard to the employees was about 14 feet.
