OSHA ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards.    On or about June 7,2021 at this work place, employees conducted water management systems operations and the employer did not have pipe system inspection and testing practices in place, to prevent pipe failures and struck-by hazards.

29 CFR  1910.22(a)(1):(1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  On or about June 7,2021 at this work place, employees conducted water management systems operations and the work areas needed    housekeeping and organization, the employees were exposed to slip, trip and fall hazards.
