29 CFR  1910.22(c):  The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:  On or about May 23, 2021, an employee was exposed to physical injury from fall hazards, while moving to and from the floor of the Storage Area and a structural beam, in order to change out the filters on a HVAC unit.  There was a 31-inch gap between the floor and the top of the beam, and the employer did not provide a safe means of access and egress to and from the upper walking-working surface.

29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer had not ensured that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about May 23, 2021, an employee was exposed to fall hazards in excess of 10 feet without fall protection, while performing work duties on the floor of the storage area above the offices.  Employer did not provide any of the following methods of fall protection: a.	Guardrail system, b.	Safety net system, or c.	Personal fall protection system.
