29 CFR  1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift:  On or about June 21, 2021, an employee was exposed to fall hazards above 15 feet to the ground below while working from a JLG Model 860SJ Articulating Boom Lift and the employer did not ensure a suitable fall protection system was used while the employee was working in the basket.
