Sources of ignition may include open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat.  a) On or about June 22, 2021, Jackson and Sons Drilling and Pump Company did not take precautions to prevent the ignition of flammable vapors in that, containers used during the transfer of category 2 flammable liquid Procool N Concentrate (85.5-99% Ethanol) were not electrically interconnected (bonded) to prevent static sparks. Transferring by means of air pressure on the container or portable tanks is prohibited.  a) On or about June 22, 2021, Jackson and Sons Drilling and Pump Company did not ensure that an approved self-closing valve was used during the transfer of category 2 flammable liquid Procool N Concentrate (85.5-99% Ethanol) from a 55-gallon drum, in the back of the truck into a plastic open lid 5-gallon container.   a) On or about June 22, 2021, Jackson and Sons Drilling and Pump Company did not ensure that an approved self-closing valve was used during the transfer of category 2 flammable liquid Procool N Concentrate (85.5-99% Ethanol) from a plastic open lid 5-gallon container into a flush cart located in the basement.
