29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; a) Jobsite: 1105 Brabec Street, Pittsburgh, PA 15212, on or about June 28, 2021, while in the process of paining the rear of a residential home, one employee was not protected from falling while working on a ladder jack scaffold. The ladder jack scaffold was set up on the ground level in the rear of the home and the employee was exposed to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.454(c)(3):When inadequacies in an affected employee's knowledge of use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee. a) Jobsite: 1105 Brabec Street, Pittsburgh, PA 15212, on or about June 28, 2021, employee that was working on a ladder jack scaffold did not have adequate training in the use of fall protection when working on the scaffold.
