29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system;  a) Jobsite: 1105 Brabec Street, Pittsburgh, PA 15212, on or about June 28, 2021, while in the process of paining the rear of a residential home, one employee was not protected from falling while working on a ladder jack scaffold. The ladder jack scaffold was set up on the ground level in the rear of the home and the employee was exposed to a fall hazard of approximately 15 feet to the ground below.
