(a) Location: 108 Bridge St, Beverly, MA 01915 a) On or about 6/24/2021, employees working from a pump jack were not protected from falling as mid-rails were missing from the pump jack scaffold, exposing employees to a fall hazard of 10 feet 8 inches. b) On or about 6/24/2021, employees working from a pump jack were not protected from falling as end-rails were missing from the pump jack scaffold, exposing employees to a fall hazard of 10 feet 8 inches.
