29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   (a) On or about June 24, 2021, employees working at the jobsite with a circular saw and other equipment requiring electrical power were exposed to electrocution, and electrical shock hazards from a powered extension cord plugged into the jobsite electrical receptacle that had the power cord protective sleeve pulled out of the crimped clamp exposing the internal wiring. The power cord did not have a strain relief mechanism.
