29 CFR  1926.453(b)(2)(iv):Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  On or about June 24, 2021, on the southeast exterior corner of a commercial structure under construction located at 8605 SE Federal Highway in Hobe Sound, Florida,  an employee performing metal framing work was exposed to a fall hazard of approximately 11 feet, 11 inches when standing on the top rail of an aerial lift.

29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about June 24, 2021, on site of a commercial structure under construction located at 8605 SE Federal Highway in Hobe Sound, Florida, an employee was exposed to a fall while performing metal framing work from the basket of an aerial lift prior to being provided with training by the employer that would enable him to recognize the hazards associated with the lift and falls.
