At this facility located at 3440 Evergreen Avenue, Jacksonville, Florida: On or about June 29, 2021, employees operated power industrial trucks (forklifts) that had the potential to tip-over, thereby exposing employees to crushed-by and struck-by hazards.

At this facility, located at 3440 Evergreen Avenue, in Jacksonville, Florida: On or about June 29, 2021, the employer exposed employees to fire and explosion hazards, in that, four LP gas cylinders were stored free standing outside the cage and were not protected from physical damage from forklift traffic.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  a. At the facility  located at 3440 Evergreen Avenue in Jacksonville, Florida: On or about June 29, 2021, the employer exposed employees to fire and explosion hazards, in that  four (4) oxygen cylinders were stored directly with two (2) fuel-gas (acetylene) cylinders, without any form of separation.
