29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about June 28, 2021, employees were observed working on roof of a church at a job site located at 852 W Bath St, Cuyahoga Falls, OH 44223. The employer did not ensure exposed employees were protected from falls by utilizing a PFAS or protected by any other form of fall protection system at an approximate fall height of 20.7 ft.
