29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) Southeast side of building - On or about June 24, 2021 and at times prior thereto, employees were accessing the scaffold, to be used as a ladder, from the flat roof approximately 3-4 feet above the scaffold platform.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Building roof - On or about June 24, 2021 and at times prior thereto, employees were working from a flat roof without fall protection.  Employees had not received specific training in fall protection, only having the general OSHA 10 hour construction course.
