29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a) MedStar EMS LLC located at 1375 Pritchett Industrial Blvd, Austell, GA 30168:  On or about June 11, 2021, employees were exposed to respiratory and bloodborne pathogen hazards in that the employer did not certify that a written hazard assessment had been performed to determine the appropriate personal protective equipment for protecting employees from hazards specific to their workplace.

29 CFR  1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) MedStar EMS LLC located at 1375 Pritchett Industrial Blvd, Austell, GA 30168:  On or about June 11, 2021, the employer exposed employees to respiratory hazards in that employees who wore N95 and KN95 respirators voluntarily were not provided with the information contained in Appendix D about the respirators' appropriate use and maintenance.
