Employees used a water pump with flexible cord as a permanent electrical installation. The water pump was operated occasionally by maintenance. This condition exposed employees to electrical hazards.

Employees used a water pump with flexible cord, which was not installed with proper strain relief, and the insulation had partially pulled away from the plug, exposing electrical wires The water pump was operated occasionally by maintenance. This condition exposed employees to electrical hazards.

Employees used hazardous chemicals to disinfect surfaces for COVID-19, but employees had not been trained on the hazards or the specific procedures the employer has implemented to protect employees from exposure. Hazardous chemicals employees were exposed to included, but were not limited to: PUROXIDE (serious eye irritation) EnvirOx H2O2 Orange Cleaner Hyper-Concentrate (serious eye damage, skin sensitization) This condition exposed employees to chemical hazards.
