Employees used a water pump with flexible cord as a permanent electrical installation. The water pump was operated occasionally by maintenance. This condition exposed employees to electrical hazards.

29 CFR  1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a)Juniper Park Limited Partnership at 2205 W 29th Avenue in Denver, Colorado 80211: On or before July 1, 2021, the employer did not ensure flexible cords were connected to devices and fittings so that strain relief was provided. Employees used a water pump with flexible cord, which was not installed with proper strain relief, and the insulation had partially pulled away from the plug, exposing electrical wires The water pump was operated occasionally by maintenance. This condition exposed employees to electrical hazards.

Employees performed tasks that exposed them to blood or Other Potentially Infectious Materials (OPIM) regularly, including administering insulin and other injections. Employees also performed housekeeping tasks and response to spills of blood or OPIM. This condition exposed employees to the hazard of contracting Hepatitis B.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Juniper Park Limited Partnership at 2205 W 29th Avenue in Denver, Colorado 80211: On or before July 1, 2021, the employer did not provide employees with effective information and training on hazardous chemicals in their work area. Employees used hazardous chemicals to disinfect surfaces for COVID-19, but employees had not been trained on the hazards or the specific procedures the employer has implemented to protect employees from exposure.  Hazardous chemicals employees were exposed to included, but were not limited to: PUROXIDE (serious eye irritation) EnvirOx H2O2 Orange Cleaner Hyper-Concentrate (serious eye damage, skin sensitization) This condition exposed employees to chemical hazards.
