29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards.  a)	Merlie Eicher dba Merlie's Post Buildings worksite located near 2550 Tracy Road, Northwood, Ohio 43619:  On or about June 30, 2021, the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.

Merlie Eicher dba Merlie's Post Buildings worksite located near 2550 Tracy Road, Northwood, Ohio 43619:  On or about June 30, 2021,  the employer did not ensure a ladder that was provided to employees at access point where there was a break in elevation of more than 19 inches. The employees were exposed to a fall distance of approximately 17 feet.
