29 CFR 1910.38(c)(2):Employer did not follow their own written procedures for type of emergency evacuation. a) Taco Bell, 9 River Street, Cortland, NY 13045, on or about 7/1/21: Store management disregarded a warning alarm, did not follow established evacuation protocols, resulting in employee exposure to fire, explosion and toxic chemical hazards. Management directed employees to reset a warning alarm, open a back door, and continue normal operations.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Taco Bell, 9 river Street, Cortland, NY 13045, on or about 7/1/21: Employees were not provided with information and training on the hazardous chemicals, including Hydrogen Sulfide, Carbon Monoxide, and Natural Gas. Specifically, training was not provided on the methods and observations used to detect the presence or release of a hazardous chemical in the work area, including continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, the physical, health, and asphyxiation, hazards of chemicals in the work environment and the measures employees can take to protect themselves from these hazards, including appropriate work practices and emergency procedures.
