29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a. On or about June 24, 2021an employee was working atop a mezzanine with unprotected sides/ edges, without the use of conventional fall protection; therefore exposing the employee to a 12'-10" fall hazard.

On or about February 17, 2021, an employee was operating a Powered Industrial Truck to lift/ move palletized loads and to lift/ raise employees onto a mezzanine, during which time the operators driving performance had not been evaluated and/ or had not been evaluated by an "Experienced Person/ Trainer" within the preceding three years; therefore exposing the employees in the workplace to struck-by hazards.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:  a. On or about February 17, 2021, an employee was operating a Powered Industrial Truck to lift/ move palletized loads and to lift/ raise employees onto a mezzanine, during which time the truck had not been examined in conformance with the manufacturer's recommendations, so as to identify conditions affecting the safety of the vehicle and wherein the truck's side/ rear-view mirrors were broken/ missing; therefore exposing the employees in the work area to struck-by and fall hazards.
