OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards from a truss collapse:  a) On or about July 7, 2021 or prior thereto: Employees were constructing a concrete walled industrial building with an eave height of approximately 22 feet and 65 feet long truss spans. The wood trusses were 2:12 slope. Only six (6) out of forty seven installed trusses had temporary diagonal bracing located near the southwest side of the structure.     b) On or about July 7, 2021 or prior thereto:  Employees were constructing a concrete walled building with wood trusses which had an eave height of approximately 22 feet.  The 65 feet long trusses had been lifted into place without the use of a spreader bar.  NOTE:  Abatement certification AND supporting documentation are required for this item.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) On or about July 7, 2021 or prior thereto: Employees were installing 65' long trusses on a precast concrete building with an eave height of approximately 22 feet without the use of fall protection.    Note - Abatement certification and documentation are required for this item.
