Timberline Excavation S Corp, at Meadow Drive and Willits Lane, Basalt, CO 81621: On or before July 7, 2021, the employer did not instruct employees in the recognition and avoidance of pinch points when assembling trench boxes, in that an employee who was handling and inserting the spreaders into a sidewall panel of a Trench Shoring shield system, model 624-4DW HE, had both of his thumbs amputated. This condition exposed employees to lacerations, crushing, and amputation hazards.   b.	Timberline Excavation S Corp, at Meadow Drive and Willits Lane, Basalt, CO 81621: On or before July 7, 2021, the employer did not instruct employees in the recognition and avoidance of pinch points when handling components such as spreaders, sidewall panels, keepers, and locking pins during assembly of a Trench Shoring shield system, model 624-4DW HE. Employees assembled this trench box on multiple occasions before the amputation incident. This condition exposed employees to lacerations, crushing, and amputation hazards.
