29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) Production Floor, Furnace #4 - On or about June 30, 2021, an employee received a left thigh injury when a pipe slipped from a boom attached to CAT Lift Truck #37 and used to remove the pipe from furnace #4.  The employer performed an operation with a modification and/or addition that affected the safe operation of the powered industrial vehicle without the manufacturer's prior written approval.

29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a) Work Floor, Furnace #4 - On or about June 30, 2021, the employer failed to ensure a powered industrial truck equipped with a front-end boom attachment was marked to identify the attachment, and the weight of the truck and attachment combination, at maximum elevation with the load laterally centered.
