29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Production Floor, Furnace #4 - On or about June 30, 2021, an employee received a left thigh injury when a pipe slipped from a boom attached to CAT Lift Truck #37 and used to remove the pipe from furnace #4. The employer performed an operation with a modification and/or addition that affected the safe operation of the powered industrial vehicle without the manufacturer's prior written approval.
