29 CFR  1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about July 8, 2021 personal fall arrest systems were not rigged to limit the fall distance to 6 feet or less, exposing employees to injuries from falls of approximately 12.5 feet.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about July 8, 2021 the employer did not verify training compliance for employees performing commercial construction operations at heights exceeding six feet.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
