29 CFR 1926.703(b)(6):Base plates, shore heads, extension devices, and adjustment screws were not in firm contact, and secured when necessary, with the foundation and the form: On or about July 02, 2021, the employer is failing to protect employees from struck-by and crushed-by hazards. The employer did not ensure the steel footing/wall form (8' x20' Efco Form) was secured as the form had been stripped of all bolts several days prior to the incident. An employee was going to pump rainwater out of pier excavation, when he threw submersible pump in the water the form fell toward him and he was caught between the form and the slope of excavation.
