29 CFR 1910.178(l)(1)(ii):  Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training.  On 7/08/2021, Power Warehouse LLC had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.  Employees operated Clark OSX15 Electric Order Pickers Serial numbers OSX15-0641-9798Kf and OSX15-0217-9798KF without having received required training.
