On or about July 14, 2021, the employer did not ensure that machine specific energy control procedures were developed, documented, and utilized when employees performed servicing and maintenance covered activities on the ZP500 High Speed Pillow Automatic Packer, also known as the Flow Wrapper machine, such as, but not limited to unjamming and cleaning activities.

On or about July 14, 2021, the employer did not ensure that employees performing servicing and maintenance covered activities on the, ZP500 High Speed Pillow Automatic Packer, also known as the Flow Wrapper machine, such as, but not limited to unjamming and cleaning, were provided with authorized lockout/tagout training.
