29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  JOB SITE:  1037 Trelane Avenue, Saint Louis, MO  63126, on or about  7/14/2021,  Workers were observed on a 6:12 pitch roof, approximately 11' 9" above ground level,  actively installing new roof shingles and operating pneumatic nail guns without any form of eye or face protection.  The workers were exposed to a struck-by hazard.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE:  1037 Trelane Avenue, Saint Louis, MO  63126, on or about  7/14/2021,  Workers were observed on a 6:12 pitch roof, approximately 11' 9" above ground level,  actively installing new roof shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were wearing PFAS, but were not tied down to an anchor plate with lanyard.   The workers were exposed to a fall hazard.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   JOB SITE: 1037 Trelane Avenue, Saint Louis, MO  63126,  on or about 7/14/2021, workers were observed using an extension ladders for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employees were exposed to a fall hazard.
