29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On July 15, 2021, located on top of a cupola on building 317 of job-site address 5330 S. Milford Rd. Milford, OH 45150,  employees were exposed to a fall hazard while installing shingles onto a steep cupola roof not utilizing any fall protection system. Potential fall height measured 12'-6 with a roof pitch of 10 in 12.

29 CFR 1926.502(a)(2): Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection. (A) Guardrail systems;  (B) Safety net systems; or  (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  On July 15, 2021, located on top of a cupola on building 317 of job-site address 5330 S. Milford Rd. Milford, OH 45150, employees were exposed to a fall hazard while installing shingles on a steep cupola roof not utilizing any fall protection system in that the employer failed to install a fall protection system prior to the employees performing the work. Potential fall height measured 12'-6" with a roof pitch of 10 in 12.
