29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about July 14, 2021, an employee used a pneumatic nail gun without safety glasses.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   On  or about July 14, 2021, employees engaged in framing activities used a multi-use ladder in the A-frame position to access the wooden frame above requiring employees to climb onto the frame.
