29 CFR  1926.451(g)(4)(iv):Midrails were not installed at a height approximately midway between the top edge and of the guardrail system and the platform surface.  a) Scissor Lift: An employee was exposed to a fall hazard of approximately 20 feet, while operating a scissor lift without the chain  installed as part of the midrail. The employee was operating a JLG scissor lift, on or about 7/14/21.

29 CFR  1926.501(b)(1):Employees on walking/working surfaces with an unprotected side and/or edge which is 6 feet or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:  a) Wendy's Restaurant/Lyndhurst NJ: An employee was exposed to a  fall hazard  of approximately 20 feet, while  installing sheet metal from a roof with unprotected side edges, the employees was screwing sheet metal on the parapet wall. The parapet wall is about 18 Inches height, on or about 7/14/21.
